You will assume that you are employed by a local Health Department as a public health professional. You have been approached by a funder to submit a proposal for a grant of $100,000 for a 12-month program that will focus on improving the health of the community using the information on the website. You will use the Generalized Model for the planning of your program.
TO DO :
1) Pick a health topic identified as being needed in the community ( FROM THE WEBSITE https://www.livehealthyfairfax.org/tiles/index/display?id=299401481862445556 )
EXAMPLE: An identified need might be that that county breast cancer death rate is higher than the state or national rate.
After you identify a specific health topic (or problem), review the Community Guide website (or similar) to identify an evidence-based strategy or intervention. This will be the focus of your proposal.
2) Find at least 5 references that you will use in developing your rationale. Your references will be reviewed for the variety of sources you are using, the titles of the sources you are using and the relevancy of the information to your topic. One reference will be the community health needs assessment or community health improvement plan from the community you chose.
3)Write a 2 page rationale. This will include background and data on the health topic, a descriiption of the community of interest, why the health topic is a problem, how you propose to address the health topic (evidence-based strategy), a related health behavior theory and expected outcomes/benefits.
