Community Assessment Presentation DIRECTIONS Part 1: This presentation is expected to be no more than ten (10) slides in length (including the title slide, objectives slide and reference slide). Include specific outcomes that you expect the community partner to learn as a result of your presentation. Include graphics and content to is relevant to your community. Personalize this as if you were to give this presentation at a community gathering. Introduction: This should catch the reader’s attention with interesting facts and supporting sources to include significant statistics. An introduction lets the reader know what to expect in the presentation. The core of your presentation (5 Slides approx.): Include components from your online research and the community assessment to express the current state of your community. Include the problem you identified in your community and include the Health People and SDOH applicable. Include a potential plan to mitigate the problem. What sort of community partnerships or interventions might be needed. Conclusion: The summary paragraph of your paper should include a statement about the problem, the population at risk for this problem and the major direct or indirect factors that contribute to this problem. This information should be no more than one or two paragraphs. References: Should include all cited references within the presentation. Include a minimum of two peer-reviewed sources and a minimum of 2 organization sites. Presentation will be submitted to the appropriate drop box by the due date designated in Module 8. The presentation will be checked by Turnitin for similarity so be sure to maintain proper academic integrity. All images should be cited properly. Part 2: Complete the Community Health Practicum Verification Hours Form. Within this form, document all activities completed in this practicum course and note the dates and time it took to complete. You must document 30 hours of activities for this practicum course to meet accreditation standards. Submit the completed Verification form in its designated drop box.
