There is no specific date on which the scenario occurred. All groups have the same sentinel event scenario.
Complete
the entire assignment on the template. Follow the directions on page 1
of the template. There will be no further updates or modifications to
the template. If you can’t locate information to respond to a
question or are unable to deduce responses to a question based on the
scenario, then enter “unknown” or “N/A” it is better to not assume an
answer. Since you first complete the template with your information,
then discuss with your group, your group should reach consensus on all
responses in creating the final product.
Add an introduction and a
summary. Refer to the rubric on page 12 of the syllabus. Put the
Introduction at the beginning before the table. Use APA format. If you
can type the summary at the end, enter it there, otherwise, label what
you have typed at the top separately as Introduction & Summary.
You
decide if the Root Cause Analysis Findings (your statement of what
happened) response contributed to the Root Cause and what would be the
Plan of Action. That drills down and carries over to Page 10- Action
Plan. You decide on Root Cause(s) based on the information you glean and
fill-in. Each recommendation should be justified by whether it
is a significant finding that contributed to the root cause. Otherwise,
superfluous recommendations will minimize the strength of other more
pertinent recommendations.
You can utilize the tools available under your assigned group for information exchange and
“SMART
objectives” are part of the Action Plans. Per the rubric there are
requirements for both- 15% & 20%, respectively. In that template
cell denote 2 labels- 1st label Action Plan, then type an
action plan, 2nd label Smart Objectives, then type a Smart .Objective.
SMART Objectives are written as sentences since they contain a verb.
