This is a proposed service improvement project on the oral care of older patients in a palliative care setting in a local hospital.It is a comprehensive written project plan on the re design of an element of service provision that enhances the experiences and outcomes of those involved in the service.You are going to design a service improvement project and use evidence based to provide the background and rationale for the project which will include QI tools, including the PDSA Cycle, appendices and Extractio table.
