Scenario:
Mrs. Thompson is an 82-year-old woman who is being admitted to a skilled nursing facility after a recent hospitalization for pneumonia. She has a history of chronic obstructive pulmonary disease (COPD), hypertension, and mild dementia. Mrs. Thompson is currently weak and fatigued, and she is experiencing difficulty walking and performing daily activities such as bathing and dressing. Instructions:

2. Diagnosis: Based on the assessment findings, formulate a nursing diagnosis for Mrs. Thompson. Explain how you arrived at this diagnosis and what key signs and symptoms support it. You may identify more than one diagnosis if appropriate.
