2024Written Assignment #2
Writing Prompt: Applying the Nursing Process (ADPIE) to Patient Care
Scenario:
Mrs. Thompson is an 82-year-old woman who is being admitted to a skilled nursing facility after a recent hospitalization for pneumonia. She has a history of chronic obstructive pulmonary disease (COPD), hypertension, and mild dementia. Mrs. Thompson is currently weak and fatigued, and she is experiencing difficulty walking and performing daily activities such as bathing and dressing. She is also mildly confused, often forgetting where she is and what day it is. During the admission assessment, you notice that she has a small pressure ulcer on her lower back, which she says has been bothering her.
As a nurse at the facility, you will need to use the Nursing Process to assess Mrs. Thompson’s condition, make a nursing diagnosis, plan appropriate interventions, implement those interventions, and evaluate her progress to ensure she receives comprehensive and individualized care.
Instructions:
Write a paper in which you explain how you would apply each step of the Nursing Process (Assessment, Diagnosis, Planning, Implementation, and Evaluation) to Mrs. Thompson’s care. Your paper should include:
1. Assessment: Describe the information you would collect during the assessment phase and why it is essential for developing an effective care plan for Mrs. Thompson. Consider her physical, cognitive, and emotional needs.
2. Diagnosis: Based on the assessment findings, formulate a nursing diagnosis for Mrs. Thompson. Explain how you arrived at this diagnosis and what key signs and symptoms support it. You may identify more than one diagnosis if appropriate.

5. Evaluation: Explain how you would evaluate Mrs. Thompson’s progress towards the goals set in the care plan. Discuss the importance of ongoing evaluation and how you would adjust the care plan if the desired outcomes are not being met.
