Case Conceptualization Report 3: The outline, rubric and details for this paper will be provided in class. The purpose of these assignments is to expand a student’s understanding of clients by conceptualizing a client based on four different counseling theories across the semester. This assignment will require you to apply 1) Cognitive Behavioral
Once you select from the forced choice of theories for each assignment, please complete both the table & integrative narrative statement. Here are the expectations for each:
The table should be completed first and is where you insert brief and bulleted ideas and concepts related to the 5 Ps of a clinical formulation and initial ideas for developing a treatment plan. No complete sentences should be used in the table.
The narrative statement should be where you take the concepts and ideas from the table and translate those to complete sentences and paragraph form. A goal for this writing is to remain brief (1-2 sentences for presentation, precipitant, pattern & perpetuants). The bulk of your clinical formulation should focus on the predisposition which you should aim to describe in 5-6 sentences.
The treatment plan table should follow the exemplar to achieve concise, clinical writing expectations. Identify 1 first order and 1 second order goal to include in this table.
